What to Bring
When checking in to our dental office, don't forget:
- Your photo ID such as a driver's license or state ID
- Completed patient forms if it's your first visit (to expedite check-in, else these can be completed at the office during check-in)
- A form of payment* and/or insurance card
- Note: We also accept CareCredit and Sunbit financing
- Any recent x-rays and records from other offices if available
What to Expect
During your visit, you'll:
- Be greeted by our front office to check-in
- Taken back to conduct exams and/or do a checkup
- Discuss personalized treatment and next steps
- Receive dental treatment and consultation
